Collection of Personal Information
Depending on how you use this Website, Proof School may request personal information from you, such as your name, child's name and grade, address, phone number, email, birthdate and other family information. Whether you choose to give Proof School this information through the Website is completely up to you, but keep in mind that Proof School may not be able to provide you with certain services or information through the Website without this information. Below are some examples of when Proof School collects personal information on the Website.

• Admission Inquiry and Application. Those interested in applying to Proof School may submit an online admission inquiry form. Upon submission of an online admission inquiry form, both the parent and prospective student will receive email communications from Proof School with information to begin the online application process. The online application process will ask for information from both prospective students and parents, including their contact information, prospective student's name, grade, current school, gender, birthdate, interests and activities, and other family information. If a prospective student subsequently enrolls in Proof School, information provided in the application process will be included in the student's school record.
• Program and Event Registration and Information. Proof School may also provide visitors to the Website the ability to register for, or obtain more information about, a variety of school events, programs and activities. If you choose to use these online forms, you may be asked to provide the participant's name, contact information and other information relating to the event, program or activity.
• Proof School Updates. If you would like to receive news and updates about the Proof School, you can provide your email address to subscribe to Proof School's email list.
• Questions, Comments and Concerns. Proof School encourages you to contact us with any questions, comments and concerns, and the Website includes an online form for this purpose. If you use the online form, we will ask for your name, email address and your phone number.
